Job Title : LPN - Long Term Care
Assignment : 3 Months Contract
Location : Claremont, NH 03743
Shift : 11:00PM - 7a/40 hours GTD.
Compensation Break-up as per 40 Hours/Week:
Pay Rate: $49/hr.
Housing and Meal Stipend: $1240 (TAX-FREE)
W2 Pay: $720 (TAXABLE)
Total Pay: $1960/Week (Excluding Taxes)
Job description:
- Primary function is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care.
- LPN works under the supervision of DON.
Responsibilities:
- Observe each resident on a continuing basis alert to changes in mental and physical well-being; notifies the Supervisor of pertinent observations and discusses the need to notify physician and receive Resident Status Report from off-going shift and give on-coming shift a detailed report of each resident’s status and events of the shift.
- Accurately receive, transcribe, and carry out physician’s orders.
- Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system of MARs/TARs and electronic documentation.
- Order and maintain all pharmaceuticals for the units.
- Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing.
- Assign and measure nursing staff resident care consistent with abilities, job description and scope of practice and supervise the care given; and assist direct-care staff with nursing care as necessary and as time allows.
- Accompany physician on resident rounds, on their assigned unit(s), to address residents’ medical needs and to facilitate communication during 60-day review.
- Administer the provision of oxygen in accordance with facility guidelines and procedures.
- Administer gastrostomy/jejunostomy feedings in accordance with policy and procedure.
- Perform wound care and have knowledge of wound vacs.
Requirements:
- 3 year current experience
- Must be willing to float
- IV CERTIFIED - Preferred
- Previous travel experience – Highly Preferred
- Rural/small facility experience – Highly Preferred
Certification Requirement:
- BLS (AHA)
State License Requirement:
- New Hampshire
- Compact Allowed
Pay: $1,920.00 - $1,960.00 per week
