Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or wheelchairs.
- Monitor and observe patients’ conditions regularly, recording vital signs like blood pressure, pulse, temperature, and respiration to ensure health stability.
- Administer medications accurately following prescribed schedules while maintaining detailed medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Provide specialized care for individuals with dementia or Alzheimer’s disease by implementing memory care strategies and ensuring a safe environment.
- Support clients with medical procedures such as catheter care or hospice-related needs, always adhering to HIPAA regulations to protect patient privacy.
- Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary; perform light housekeeping tasks including cleaning and laundry to maintain a safe living space.
- Use proper techniques for patient transfers and mobility assistance utilizing tools like Hoyer lifts or transfer belts to prevent injury.
Requirements
- Proven experience working in home health care, assisted living facilities, nursing homes, or long-term care environments.
- Knowledge of medical terminology, medication administration protocols, and vital signs monitoring.
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus.
- Ability to work independently while maintaining excellent communication with healthcare teams; familiarity with EMR/EHR systems is preferred.
- Compassionate attitude toward working with individuals with disabilities or developmental disabilities; experience in memory care or Alzheimer’s care is highly valued.
- Strong organizational skills for managing medical records, documentation, and patient schedules while ensuring compliance with HIPAA standards.
