Position Summary: The Clinical Care Nurse is a Licensed Vocational Nurse (LVN) who serves as a clinical resource supporting assigned primary care and pediatric providers through efficient care coordination, clinical communication, and management of provider workflows across TAN Healthcare's Beaumont and Orange locations. This position promotes timely access to care by coordinating medication refills, prior authorizations, diagnostic testing, durable medical equipment (DME), and other clinical tasks from initiation through resolution.


This position primarily provides clinical support through provider workflow management and care coordination while also providing direct patient care, triage support, and other clinical nursing duties as operational needs require to ensure safe, efficient, and patient-centered clinic operations.


Essential Job Responsibilities

Urgent Clinical Needs and Same Day Response

  • Monitor and respond to provider and patient clinical messages, refill requests, order clarifications, and other time-sensitive clinical needs within assigned workflows.
  • Provide backup telephone and walk-in triage within LVN scope of practice, standing orders, organizational policies, and established clinical protocols.
  • Recognize red flags and immediately escalate urgent or high-risk patient concerns to the appropriate provider or clinical leader.
  • Promptly communicate urgent laboratory or diagnostic results, medication concerns, delays in care, or significant changes in patient condition to the appropriate provider.

 

Provider Orders, Authorizations, and Access to Care

  • Complete prior authorizations for medications, imaging, procedures, durable medical equipment (DME), specialty services, and other ordered care.
  • Gather and submit required clinical documentation through payer-specific authorization systems.
  • Coordinate DME orders and diagnostic imaging while ensuring diagnoses, provider documentation, insurance information, and medical necessity requirements are complete.
  • Communicate authorization approvals, denials, appointments, preparation instructions, vendor requirements, and next steps to patients, providers, pharmacies, vendors, and outside healthcare facilities.
  • Support denial follow-up, appeals, peer-to-peer reviews, and alternate treatment plans as directed by the provider.

 

Care Coordination, Tracking, and Documentation

  • Maintain organized work queues for pending messages, orders, authorizations, DME, imaging, and other assigned clinical tasks.
  • Follow each assigned item through approval, completion, denial, or documented handoff to ensure continuity of care.
  • Follow up on missing information, delayed requests, missed appointments, expiring authorizations, and outstanding results while proactively identifying barriers to care.
  • Accurately document all patient communication, authorization activity, clinical actions, follow-up efforts, and next steps within the electronic health record.

 

Cross Site Clinical Support

  • Coordinate daily with assigned primary care and pediatric providers and clinical teams across Beaumont and Orange locations to maintain efficient patient care workflows.
  • Provide direct clinical support, including medication administration, laboratory assistance, patient education, immunizations, and other nursing duties within LVN scope of practice as assigned and operationally necessary.
  • Assist with clinic coverage during periods of increased patient volume, staffing shortages, or other operational needs.
  • Participate in daily huddles, staff meetings, competency assessments, quality improvement initiatives, and organizational training.
  • Comply with all TAN Healthcare policies and procedures, including HIPAA, OSHA, infection prevention, patient safety, and regulatory requirements.
  • Perform other duties within the scope of licensure as assigned.



Minimum Education, Qualifications, and Experience

Required

  • Graduate from an accredited Vocational Nursing program.
  • Current, unrestricted Texas Licensed Vocational Nurse (LVN) license.
  • Current Basic Life Support (BLS) certification for Healthcare Providers.
  • Minimum of one (1) year of LVN experience in primary care, specialty care, community health, or a similar ambulatory setting preferred.
  • Experience with medication refills, prior authorizations, durable medical equipment (DME), diagnostic imaging coordination, insurance requirements, and electronic health records strongly preferred.
  • Experience using eClinicalWorks (eCW) is preferred.
  • Knowledge of primary care and pediatric clinical workflows is preferred.
  • Valid Texas driver's license, reliable transportation, and ability to routinely travel between TAN Healthcare clinic locations.

Physical Requirements

  • Ability to sit, stand, walk, bend, reach, and move throughout the clinic environment for extended periods.
  • Ability to frequently operate a computer, telephone, and other standard office and clinical equipment.
  • Ability to communicate effectively in person, electronically, and by telephone.
  • Ability to occasionally lift and move up to 25 pounds.
  • Ability to safely perform duties requiring manual dexterity and visual acuity.
