What are the responsibilities and job description for the Registered Dietitian / Clinical Nutrition Health Coach position at Tria Health?
Summary: Provide clinical expertise in prescriptive nutrition guidance for complex chronic condition management. Collaborate with certified health coaches, clinical pharmacists, and other staff to deliver patient-centered, provider-led, and technology-enabled care that drives optimal health outcomes.
Essential Duties and Responsibilities:
- Act as a clinical nutrition advisor to certified health coaches and clinical pharmacists.
- Collaborate with Manager of Health & Wellness Programs to develop and implement prescriptive nutrition protocols for patients with complex chronic conditions.
- Provide education and consultations to eligible members of the Tria Health program.
- Complete consultations, outlining program requirements and ensuring patient understanding of Choose to Lose (Tria’s weight loss program).
- Collaborate with Clinical team members to facilitate case reviews and contribute to care-planning for complex chronic disease management cases.
- Monitor patient progress and adjust nutrition plans based on goals and outcomes.
- Lead continuing education initiatives for Clinical department team on nutrition and chronic disease management.
- Provide continuity of care to the patient load assigned.
- Assist with development of Tria Health’s nutrition-related programming, including app content, educational materials, and patient engagement strategies.
- Serve as a clinical resource for members, clients, brokers, Third Party Administrators, peers, and coworkers.
- Assist with training of coworkers from other departments upon request.
- Offer evening appointment availability to accommodate patient needs.
- Work in other functional areas to cover absences or relief, to equalize peak work periods or otherwise to balance the workload.

Education/Experience:
- Required: Bachelor’s degree in nutrition or related healthcare field.
- Preferred: Master’s degree in nutrition or related healthcare field.
- Required: Registered Dietitian (RD or RDN).
- Preferred: Certification in Health Coaching (NBC-HWC) or CDE (Certified Diabetes Educator).
- Required: 2–3 years of direct patient care experience in nutrition counseling, including the development and implementation of individualized clinical dietary prescriptions for chronic disease management.
Job Competencies:
- Read, analyze, and interpret general business periodicals, professional journals, technical procedures, governmental regulations, and clinical literature.
- Write reports, business correspondence, and procedure manuals.
- Effective communication skills are required to succeed in this position.
- Solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists is desired.
- Interpret a variety of instructions furnished in written, oral, diagram, or scheduled form.
- Intermediate to advanced knowledge of Word, Access, Excel, Database software, E-mail, and Internet.

- Work Environment: This job operates in a professional office environment where standard office equipment such as computer, phones, copiers, filing cabinets, and printers are utilized. The noise level in the work environment is usually minimal.
- Location: Tria Health is located at 1729 Grand Boulevard, Kansas City, MO (Crossroads area). This is not a remote job position, but a hybrid schedule can be utilized after training.
- Hours: 40-hour workweek, Monday – Friday with 1 evening shift (12:30-9).
