Job Overview
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. Responsibilities
- Conduct thorough physical examinations and health assessments for diverse patient populations, including children, adults, and seniors.
- Manage acute and chronic conditions by developing and implementing individualized treatment plans, including medication administration, injections, and suturing.
- Utilize telehealth platforms to provide remote consultations and follow-up care, ensuring accessibility for all patients.
- Coordinate with multidisciplinary teams to facilitate discharge planning, case management, and utilization review across inpatient and outpatient settings.
- Maintain accurate medical documentation using EMR system.
- Engage in patient education and health coaching to promote wellness, disease prevention, and management of complex conditions like infectious diseases or geriatrics.
Experience
- Proven experience as a Family Nurse Practitioner in diverse clinical environments such as hospitals (including Level I Trauma Centers), urgent care clinics, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR system.
- Hands-on experience performing procedures like suturing, phlebotomy, catheter care, venipuncture, and sterile processing techniques.
- Critical care experience in ICU or emergency medicine settings is highly desirable.
- Background working with special populations such as pediatric patients (including neonatal ventilator management), geriatric patients (memory care/dementia), or individuals with developmental disabilities.
- Knowledge of CPT coding, ICD-10/ICD-9 coding systems for accurate billing and documentation.
- Familiarity with medical imaging techniques like sonography or laboratory specimen collection/processing is a plus.
- Experience in telehealth delivery models to expand access to quality care across various settings including home care or assisted living facilities.
