Urgent Care and Skilled Nursing / Rehab – Full-Time

Position Overview
We are seeking two full-time Nurse Practitioners or Physician Assistants for a structured hybrid role combining Urgent Care and Skilled Nursing Facility (SNF) / Rehabilitation rounding.
This position offers a unique opportunity to gain experience in both acute care and post-acute medicine within a predictable and well-organized schedule. Providers will develop a broad clinical skill set across multiple care settings while maintaining excellent work-life balance.
Schedule
Full-time position (compensation-based)
Approximately 71 clinical hours every 2 weeks
No more than 4 consecutive workdays
No after-hours responsibilities unless on call
Coverage Structure:
Urgent Care: Tuesday, Thursday, and alternating weekends (8:00 AM – 5:00 PM)
Nursing Home / SNF: Monday, Wednesday, Friday (9:00 AM – 4:00 PM)
Compensation and Benefits
Salary: $160,000 per year
Sign-on bonus: $5,000
Relocation assistance: $5,000

Clinical Responsibilities
Urgent Care:
Evaluate and treat acute illnesses and injuries
Perform procedures and point-of-care testing
Order and interpret labs and imaging
Skilled Nursing / Rehab:
Round on skilled nursing, long-term care, and rehab patients
Perform follow-ups, and chronic care management
Conduct Transitional Care Management (TCM) visits
Coordinate Chronic Care Management (CCM) with support staff
Complete documentation in EHR
Patient Volume
Average approximately 21 patients per day across settings
Additional Information
Travel between clinical sites is required
Providers do not need to return to the nursing facility after completing daily rounds
Strong administrative and clinical support in place
Qualifications
Licensed Nurse Practitioner or Physician Assistant in Pennsylvania
Experience in Urgent Care, Emergency Medicine, Internal Medicine, or SNF/LTC preferred
Strong clinical and documentation skills
Ability to work independently and manage time efficiently
