Job Summary
The Home Health Physical Therapist will be responsible for providing physical therapy services to patients in their homes. Responsibilities
- Conduct initial assessments of patients to determine their physical therapy needs.
- Develop treatment plans based on patient needs and goals.
- Provide physical therapy services to patients in their homes.
- Monitor patient progress and adjust treatment plans as needed.

Requirements
- Current state license as a Physical Therapist
