JOB SUMMARY:
Support the growth and development of Tri-County Health Network’s (TCHNetwork) programs by providing direct care coordination to Rocky Mountain Health Plan Medicaid members and the community. Facilitate calls, coordinate care based on client needs, and expand TCHNetwork’s community partnerships.
DUTIES AND RESPONSIBILITIES:
- Serve as the primary contact for Medicaid members in the community.
- Conducted client assessments and developed care plans.
- Educate and assist clients with preventive and disease management services.
- Document client interactions and ensure timely completion of reports.
- Facilitate care coordination meetings.
- Educate clinic and partner organization staff about care coordination services.
- Communicate TCHNetwork’s mission to community groups.
- Manage incoming referrals.
- Attend regional meetings, conferences, and training sessions.
- Assist with developing resource lists with the Regional Health Connector.
LOCATION:
Hybrid position in Montrose. With travel throughout the region.
SALARY & BENEFITS:
Starting salary: $19.00 - $22.00 per hour, based on experience.
