Job Overview: The role of the Care Manager is to work collaboratively with individuals with intellectual/developmental disabilities, their families, OPWDD, and relevant providers to ensure that all individuals supported are able to live safe, happy, and fulfilling lives. This includes working to identify goals, building plans to reach those goals, and assistance with applying to a variety of supports and services to put the individual on the right path to meet their person-centered outcomes. Tri-County Care supports members of all ages – this may include advocating for individuals with their schools, helping them to find medical providers, assisting them with applications to Social Security, SNAP benefits, Waiver programs, etc.

Location: This position is primarily remote; however, it requires periodic in-person visits with individuals and families. The caseload for this role is based in Manhattan and Staten Island, and candidates must be able to travel to these locations for these visits using a car or public transportation. While the job may be posted in New Jersey, the work itself is tied to Manhattan and Staten Island, New York.
This position requires applicants outside of New York City to have a valid driver's license and reliable transportation. Travel reimbursement is provided.
This position requires a quiet distraction-free environment for working, or the ability to work from one of our regional offices.
