Working under the supervision of a Care Manager, the Associate helps deliver the six core services of Care Coordination in a person-centered, compliant, and collaborative manner. This role ensures that each individual’s needs, preferences, and goals are reflected in their Life Plan while supporting OPWDD valued outcomes, People First Transformation objectives, and all state requirements.
