What are the responsibilities and job description for the Adult Mental Health Specialist position at Tri-County Behavioral Healthcare?
The Adult Mental Health Specialist provides ongoing monitoring, recovery planning, skills training, and mental health case management to adults with chronic and persistent mental illness.
Where you will work
This is a community mental health position with the office as the primary work location. Staff typically begin and end their workday at the office and provide services in clients’ homes and other community settings across Liberty County. Work environments vary and may include rural residences, shelters, public spaces (libraries, parks, outdoor areas), transitional housing programs (Salvation Army, Compassion United), and conditions are not always predictable or structured.
What you’ll do
- Provide psychosocial rehabilitation skills training and community based mental health case management
- Support goals identified in each person-centered recovery plan
- Locate, engage, and meet with individuals on your caseload
- Coordinate with providers and community resources
- Complete documentation within required timeframes in the electronic health record.
What success looks like in this position
Success in this position requires persistence, flexibility, and follow through to stay connected with clients who may have changing schedules and field environments. Staff are comfortable making repeated outreach attempts (phone calls, home visits, checking known locations or community sites), adjusting plans throughout the day, and continuing engagement efforts when individuals are difficult to reach.
Effective staff remain calm and respectful when working in varied home and community environments and when supporting individuals with complex behavioral health needs. Staff work independently with their caseload while maintaining communication with team members, staying actively engaged with individuals served, and consistently meeting service and documentation expectations.
