POSITION SUMMARY
This position provides comprehensive heart failure monitoring, education and patient management per protocol of Tri City Cardiology.
ESSENTIAL FUNCTIONS
- Ability to retrieve heart failure reports and information from multiple device programmers and multiple remote monitoring websites.
- Perform clinical assessment, interpret cardiac device information, develop a follow up plan, document and send to providers for review and recommendations.
- Maintain knowledge in various aspects of Heart Failure to ensure quality patient management.
- Provide comprehensive education to patients, families and caregivers regarding heart failure.
- Collaborate with Triage nurses, peers, staff and providers for to ensure comprehensive patient management.
- Knowledge of and referral to outside resources to assist patient with individual heart failure management needs.
- Maintain tracking and documentation to ensure quality management of high risk HF population and to support organizational quality processes.
- Primary resource for patients in the Heart Failure Clinic for non-urgent issues for the organization.
- Responds to all department emergencies as a BLS/ACLS certified employee.
MINIMUM QUALIFICATIONS
Education/Experience/Licenses/Certifications
High school diploma or GED required.
Medical assistant, certified nursing assistant, or related clinical experience
Medical assistant diploma, nursing assistant certification or clinical credentials required.
Cardiology experience required.
Current BLS certification (must be obtained in 90 days if not current)

PREFERRED QUALIFICATIONS
Previous MA (advance) experience preferred.
Six to twelve months experience in device and/or heart failure management preferred
Knowledge of basic venous anatomy preferred.
ACLS preferred
Salary : $20 - $25
