Position SummaryThe Community Marketer is responsible for developing and maintaining referral relationships with hospitals, physicians, case managers, discharge planners, community partners, and other healthcare professionals. This position works closely with facility leadership, admissions, nursing, and business office teams to support census growth, coordinate admissions, and promote the facility’s services in the community.
This role combines marketing strategy, referral development, admissions coordination, and customer service to ensure a positive experience for residents, families, and referral partners.
Responsibilities IncludeDevelops, maintains, and strengthens relationships with hospitals, physicians, discharge planners, case managers, social workers, ACOs, managed care organizations, and other referral sources.
Markets the facility’s services, programs, and clinical capabilities to referral partners and the community.
Serves as a key liaison between referral sources, residents, families, and facility staff.
Evaluates potential admissions in coordination with nursing, admissions, and facility leadership.
Communicates clinical and financial information needed to determine appropriate placement and admission eligibility.
Coordinates with appropriate medical and facility staff to schedule and support resident admissions.
Assists with bed availability, admission planning, and communication with residents and families.
Supports the admissions process by helping ensure timely communication, accurate information, and a smooth transition into the facility.
Tracks referrals, admissions, lost referrals, hospital activity, and referral trends.
Identifies opportunities to increase census, improve referral conversion, and expand facility visibility.
Participates in sales and marketing planning to support facility census goals.
Conducts facility tours for prospective residents, families, and referral partners.
Represents the facility at community events, hospital meetings, networking opportunities, and healthcare-related functions.
Provides regular updates and reports to the Administrator regarding referral activity, census trends, marketing efforts, and growth opportunities.
Assists in developing marketing materials, outreach plans, and strategies to promote facility services.
Maintains a professional, positive, and responsive relationship with residents, families, referral partners, and community contacts.
Works collaboratively with the interdisciplinary team to support resident-centered care and customer satisfaction.
Prior experience in skilled nursing, long-term care, assisted living, hospital discharge planning, healthcare marketing, admissions, case management, or clinical liaison work preferred.
Strong understanding of skilled nursing admissions, post-acute care, managed care, Medicare, Medicaid, and hospital referral processes preferred.
Must possess excellent communication, presentation, and relationship-building skills.
Must be able to positively interact with residents, families, staff, referral sources, physicians, hospital personnel, government agencies, and the general public.
Must be organized, self-motivated, and able to manage multiple priorities.
Must have strong customer service skills and a professional appearance and demeanor.
Must be comfortable with outreach, networking, follow-up, and referral development.
Knowledge of healthcare regulations, confidentiality requirements, and resident rights preferred.
Ability to track referral activity, analyze trends, and support census development goals.
Valid driver’s license and ability to travel locally for referral development and marketing activities may be required.
