What are the responsibilities and job description for the Family Service Coordinator position at Tresco Inc?
What Youll Do
Coordinate accurate intakes, evaluations, and assessments to determine eligibility
Facilitate and actively participate in the development and reviews of Individual Family Service Plans (IFSPs)
Support families in accessing community resources beyond the childs condition
Coordinate services and monitor delivery, including subcontractors and cross-agency providers
Maintain up-to-date and accurate child files in compliance with accreditation and Medicaid standards
Facilitate transition planning for children exiting the program
Gather and manage a community resource register to better serve families
Stay current through required training programs and professional development
What Were Looking For
Education: Bachelors degree in social work, Psychology, Nursing, Special Education, Early Childhood Education, Counseling, or related field
License: Valid drivers license required
Background check and driving record check required
Bilingual skills are a plus!
