Hospice RN Case Manager (Part-Time)

We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our hospice team. This role is ideal for an RN who values meaningful patient relationships, interdisciplinary collaboration, and providing end-of-life care with dignity, empathy, and excellence.
Position Details
- Schedule: 30 hours per week working Monday, Tuesday, Thursday and Friday, with Wednesday off. Daily work hours may vary based on patient and business needs.
- On-Call Requirements:
- One on-call weekend per month
- Two on-call nights per month
- Pay: $34.00–$54.25 per hour, depending on years of experience and qualifications
- Employment Type: Part-Time
Key Responsibilities
- Provide direct nursing care to hospice patients in their place of residence
- Serve as the primary case manager, overseeing and coordinating all aspects of patient care
- Manage and collaborate with interdisciplinary team members, including: LPNs, Hospice aides (bath aides), Social workers, chaplains, and other disciplines
- Develop, implement, review, and update individualized plans of care
- Complete accurate, timely clinical documentation and charting
- Follow up on care plans, including reassessments and resubmissions as required
- Communicate effectively with patients, families, physicians, and other healthcare professionals
- Provide education and emotional support to patients and families, leading with empathy and compassion
- Participate in on-call rotation to support patients after hours as needed
- Ensure compliance with hospice regulations, agency policies, and standards of care
Qualifications
- Current Registered Nurse (RN) MT license
- Hospice or home health experience preferred (but not required for the right candidate)
- Strong clinical assessment and care coordination skills
- Excellent communication, organization, and documentation abilities
- Ability to work independently while collaborating with a multidisciplinary team
- Valid driver’s license, proof of insurance coverage, and reliable transportation
- EMR experience preferred
