EARN Case Manager – Full Time (Hybrid)
Trehab Community Action Agency – Wellsboro, PA
Job type: Full-time (37.5 hours/week)
Start date: As soon as possible
Work model: Hybrid (primarily remote with 1 mandatory in-office day/week in Wellsboro). Laptop equipment provided. Candidates must have a suitable work-from-home setup that allows for uninterrupted work. Remote work hours must be treated the same as in-office hours; employees may not serve as the primary caregiver during scheduled work hours.
About Us
Trehab is a Community Action Agency dedicated to strengthening our region through free Workforce Development programs and supportive services. We are seeking a motivated, compassionate individual to join our team as an EARN Case Manager, serving clients across Tioga, Sullivan, Wyoming, and Susquehanna counties.
Our EARN Program supports individuals receiving county assistance in overcoming barriers, developing employment skills, and securing meaningful, long-term careers. Services include individualized case management, job search assistance, career readiness training, and placement into paid work experiences, on-the-job trainings, and short-term vocational programs.

Job Duties Include:
- Providing intensive case management & career counseling to clients across four counties
- Completing initial mental health assessments and identifying supportive service needs
- Helping clients set goals and navigate individualized career pathways
- Assisting clients in identifying & overcoming barriers to employment
- Conducting mock interviews, resume development, and connecting clients to training opportunities
- Entering weekly time documentation for clients & maintaining detailed case notes
- Facilitating workshops and virtual meetings
- Working both independently (remote) and collaboratively during scheduled in-office days
A Successful Candidate Will:
- Be innovative in creating new ways to engage and retain clients
- Demonstrate strong teamwork and customer service skills
- Be highly organized with proficient record-keeping abilities
- Be comfortable with Microsoft Office Suite, Google Suite, and virtual platforms
Minimum Qualifications:
- Bachelor’s Degree preferred, ideally in Social Work, Counseling, Psychology, Human Services, or a related field
- Equivalent experience may be considered
- Ability to perform basic mental health assessments is strongly preferred
- Must pass all required PA Clearances upon hire
- Valid driver’s license required
Required Knowledge, Skills, & Abilities:
- Knowledge of workforce development programs, services, and performance expectations
- Excellent customer service skills and ability to work with diverse populations
- Career counseling experience, including resume and cover letter development
- Experience with customer assessment, eligibility screening, and supportive services
- Ability to understand and follow federal, state, and local guidelines
- Strong verbal & written communication skills; ability to multi-task
- Ability to maintain confidentiality, professionalism, and credibility
- Working knowledge of Microsoft Office & Google Suite, and ability to teach basic computer skills to clients
- Technological proficiency to manage multiple systems and maintain a remote caseload
