Position Summary

The Human Services Program Specialist is responsible for conducting welfare assessments, school-based engagement, and community outreach to support program participants and their families.

This role serves as a key liaison between participants, schools, and community resources, ensuring that each individual receives structured support through assessment-driven service plans, targeted interventions, and consistent follow-up. The Specialist will identify needs, monitor progress, and implement actionable plans that promote safety, well-being, and long-term success.

Key Responsibilities

Welfare Assessments

  • Conduct ongoing participant welfare assessments to evaluate well-being, safety, and social-emotional needs
  • Utilize structured tools and professional judgment to identify risks, barriers, and support needs
  • Monitor changes in participant circumstances and escalate concerns when appropriate
  • Maintain timely and accurate documentation of all assessments

School Engagement & Monitoring

  • Conduct school visits to review academic performance, attendance, and behavioral trends
  • Build strong relationships with teachers, counselors, and school administrators
  • Advocate for participants and support alignment between educational and program goals
  • Identify early intervention opportunities based on school feedback and data

Community Engagement

  • Represent TCGC in the community through outreach events, workshops, and partner meetings
  • Build and maintain relationships with local service providers and community organizations
  • Facilitate connections between participants and community-based resources
  • Support engagement strategies that increase program participation and retention

Service Planning & Follow-Up

  • Develop individualized action plans based on assessment findings
  • Establish measurable goals, timelines, and clear follow-up activities
  • Conduct consistent check-ins and follow-up engagements to track progress
  • Adjust plans based on participant outcomes and evolving needs

Case Coordination & Resource Referral

  • Coordinate referrals to appropriate support services (mental health, academic support, social services, etc.)
  • Collaborate with internal leadership and external partners to ensure continuity of care
  • Maintain communication with participants and families to reinforce engagement

Targeted Group Support (Supplemental Intervention)

  • Facilitate small, structured group sessions designed to address specific participant needs identified through welfare assessments and school engagement
  • Utilize evidence-based tools and curricula, such as Overcoming Obstacles, to support development in areas including confidence building, conflict resolution, communication, and goal setting
  • Implement short-term, targeted interventions that complement—rather than replace—core program instruction

Scope & Parameters

  • Deliver no more than 1–2 small group sessions per week
  • Ensure all group sessions are directly tied to documented participant needs and assessment findings (not general programming)
  • Maintain alignment between group participation and individual service/action plans
  • Track and document participant progress, including:
    • Attendance and engagement
    • Behavioral improvements
    • Social-emotional learning (SEL) outcomes

Outcomes & Accountability

  • Establish measurable goals for each group intervention
  • Provide brief post-session summaries and outcome reports
  • Adjust or discontinue group interventions based on effectiveness and participant progress

Documentation & Compliance

  • Maintain detailed, confidential records in accordance with nonprofit and legal standards
  • Prepare regular reports on participant progress, outcomes, and program impact
  • Ensure compliance with safeguarding, confidentiality, and mandatory reporting requirements

Qualifications

Education & Experience

  • 2 years of experience in human services, youth programming, or community outreach
  • Experience working with schools and community-based organizations strongly preferred

Skills & Competencies

  • Strong assessment and critical thinking skills
  • Ability to develop and manage structured service/action plans
  • Excellent relationship-building and communication skills
  • Strong organizational and documentation capabilities
  • Ability to work independently in a field-based, high-accountability role

Preferred

  • Knowledge of trauma-informed care and youth development principles
  • Familiarity with Florida safeguarding and reporting requirements
  • Bilingual (Spanish or Haitian Creole) a plus

Contract Expectations

  • Independent contractor (1099) classification
  • Anticipated average of 20–25 hours per week, with potential fluctuations based on program cycles and seasonal needs.
  • Must have reliable transportation for local travel (schools, community sites, events)
  • Flexible schedule, including occasional evenings for community engagement activities
  • Responsible for maintaining professional standards and confidentiality
  • Background screening required

Compensation

  • Competitive hourly or per-engagement rate based on experience
  • Mileage reimbursement for approved travel
  • Option for caseload-based or retainer structure
