Key Responsibilities
- Provide direct patient care in accordance with established nursing standards and facility policies.
- Assist with assessments, care planning, and implementation of nursing interventions.
- Administer medications and treatments as prescribed.
- Monitor, document, and report changes in resident conditions to the RN or physician.
- Support activities of daily living, ensuring safety, dignity, and resident satisfaction.
- Collaborate with interdisciplinary teams including RNs, CNAs, therapists, and families.
- Maintain accurate documentation and compliance with state and federal regulations.
- Participate in care conferences, training, and quality improvement initiatives.
- Active LPN license in the state of Hawaii (or eligibility to obtain).
- Previous experience in Long-Term Care or Skilled Nursing preferred.
- Strong communication, teamwork, and organizational skills.
- Ability to work in a fast-paced, resident-focused environment.
- Compassionate approach and commitment to quality care.
- Familiarity with EMR charting systems.
- Experience supporting geriatric populations.
- Knowledge of state and federal LTC regulatory standards.
