Responsibilities
- Conduct patient evaluations and develop individualized treatment plans.
- Deliver therapeutic interventions aligned with patient goals, safety standards, and regulatory guidelines.
- Document all treatments, progress notes, and assessments accurately and in a timely manner.
- Collaborate with interdisciplinary teams, including nursing staff, physicians, and therapy assistants.
- Provide patient and caregiver education on therapy programs, mobility techniques, and safety.
- Participate in care planning meetings as needed.
- Maintain professional standards and adhere to facility policies and compliance requirements.
- Active and current Physical Therapist (PT) license in the state of California.
- Minimum experience: Less than 1 year accepted.
- Prior SNF experience preferred but not required.
- Strong communication, organizational, and patient-care skills.
- Ability to work independently and adapt to a fast-paced environment.
- Schedule varies based on facility needs.
- Holiday coverage is not required.
- EMR system: Not listed.
- Bilingual candidates are welcome but not required.
