Job Summary

Key Responsibilities
- Provide individualized patient care in accordance with care plans and physician orders
- Conduct patient assessments, report abnormal findings, and assist with developing care plans
- Administer medications safely and accurately, documenting per policy
- Perform clinical tasks such as EKGs, blood draws, IV starts, wound care, and specimen collection
- Respond promptly and appropriately to emergencies, following resuscitation protocols
- Maintain accurate patient documentation, including assessments, interventions, and progress notes
- Support continuity of care through effective communication with the Charge Nurse, RN, physicians, and interdisciplinary staff
- Assist with patient transfers for tests/procedures and coordinate with other departments
- Uphold infection control, safety procedures, confidentiality, and patient rights
- Assist in orienting new staff and participate in ongoing training and staff meetings
- Provide support to other nursing departments when needed
- Maintain professionalism, positive communication, and adherence to organizational policies
- Must function independently while demonstrating integrity, flexibility, and teamwork
- Active California LVN license
- Current BLS certification; ACLS preferred
- Minimum 1 year of LVN experience (SNF experience strongly valued)
- Ability to walk, bend, lift, and perform manual patient care tasks
- Strong mental resilience and ability to work with patients who may be confused or verbally/physically abusive
- Excellent verbal and written communication skills in English
- Reliable attendance and strong interpersonal skills
- Ability to follow directions, adapt to change, manage time effectively, and stay organized
- Basic computer proficiency
