POSITION OVERVIEW:
Seeking a Clinical Pharmacist to join an award-winning Pain Management /Oncology service rotating between the inpatient pain consult service and inpatient oncology unit. This pharmacist will serve as one of three clinical pharmacy experts in the area of pain and oncology management providing individualized patient pain regimen consultations and oncology services to inpatients.
RESPONSIBIITIES:
- Completes individualized patient pain assessments and develops pain regimens to ensure effective pain control for acute, chronic, break-through and neuropathic pain as consulted.
- Completes individualized assessment and monitoring of patients receiving chemotherapy medications within the acute care setting
- Oversees safe and accurate compounding and dispensing of chemotherapy medication in the acute care setting
- Participates in the formulary process for pain and oncology related medications by preparing drug monographs of requested non-formulary and/or formulary pain medications or performing medication use evaluations (MUEs).
- Participates in the development and implementation of policy and procedures, order sets and/or institutional use guidelines to promote safe and effective use of pain and oncology medications.
- Participate in interprofessional patient care with hospitalist, palliative/hospice care, oncology and anesthesia physician consultants.
- Completes opioid stewardship review on targeted patients receiving pain management therapy to ensure appropriateness and intervenes as necessary.
- Teaching/precepting PGY1 residents and pharmacy students.
- Completion of pain/oncology related patient care projects/presentations, and participation on multidisciplinary institutional committees for the pharmacy department and organization.
- Documents clinical interventions on an ongoing basis to measure the clinical and economic effectiveness of the service with formal reporting at defined intervals.
QUALIFICATIONS:
- Doctor of Pharmacy (Pharm. D.) degree required
- Completion of PGY 1 residency or at least two years acute inpatient hospital pharmacist experience as applicable to the position·
- PGY2 in pain/palliative care, oncology or equivalent experience preferred
ABOUT THE EMPLOYER:
The Inpatient Pharmacy Department provides progressive pharmacy services across three campuses and includes a staff of 160 with over 50 clinical pharmacists A full array of technology is available to assist with distribution of medications to our patients including automated inventory management, automated dispensing cabinets, bar code technology, automated TPN and workflow management systems.
CLINICAL INPATIENT PHARMACY SERVICES:
- Integrated decentralized staffing model
- Pharmacists are stationed on nursing units participating in order verification, provision of drug information, and clinical services.
- Clinical services include pharmacokinetics, anticoagulation management, and automatic intravenous to oral switches, automatic renal dose adjustments and automatic therapeutic interchanges.
- Specialty pharmacy services include Anemia Management, Anticoagulation, Critical Care, Emergency Medicine, Formulary Management, Infectious Diseases, Oncology, Pain Management, and Pediatrics.
