In this role, you’ll partner with the Community Living Manager to provide high-quality, person-centered care. You’ll support daily routines, encourage independence, and help residents achieve their personal goals—all while creating a safe, supportive, and engaging home environment.

What You’ll Do
Support & Empower Residents
- Assist with daily living activities (ADLs) such as personal care, meal preparation, and transportation
- Encourage independence by teaching and modeling life skills
- Support residents in achieving their personal goals and participating in community activities
- Help manage social interactions and provide guidance during conflicts when needed
- Administer medications and maintain proper documentation
Create a Safe & Supportive Environment
- Maintain a clean, safe, and welcoming home
- Ensure the health, safety, and well-being of all residents
- Follow all HIPAA and organizational policies and procedures
- Identify and respond appropriately to emergencies or conflicts
- Complete routine safety checks, including monthly inspections
Communicate & Collaborate
- Maintain accurate and timely documentation (logs, reports, care plans, etc.)
- Build respectful, professional relationships with residents, families, and external partners
- Communicate effectively with team members and leadership
- Assist with care planning and provide input on residents’ progress and goals

Physical Requirements
This role requires the ability to:
- Assist with personal care (including showering and toileting)
- Lift up to 50 pounds
- Push a wheelchair over uneven surfaces and inclines
- Walk, stand, bend, and move frequently throughout the day
- Assist residents in and out of vehicles
- Work in a variety of weather conditions
