Home Health PT – $75–$85 per visit – Las Cruces, NM (Lead Clinician)
Transmountain Therapy is a locally owned home health therapy contract group serving El Paso, TX and Southern New Mexico. We've been running for 3 years, with 30 therapists in our network. We're hiring a lead Physical Therapist for our growing home health operations in Las Cruces, Sunland Park, Santa Teresa, Anthony, Vado and Chaparral. This is our top-priority role, and the rate includes an urgency premium.
Pay
- $75–$85 per visit
- At 10–20 visits a week, that's about $750–$1,700 a week before taxes
- Fast pay by direct deposit
- 1099 PRN: you set your volume, from 5 to 20 visits a week
What you'll do
- Evaluate home health patients across Southern NM and set their plans of care
- Perform re-certifications and discharges
- Supervise our PTAs already working in the field
- Document evals, progress notes and plans of care in the agency's EMR (Devero, Kinnser/WellSky or Alora)
- Attend weekly case conferences with partner agencies
- Pick your weekly visit volume and zones

Requirements
- Active New Mexico PT license
- Current professional liability/malpractice insurance
- Valid driver's license and reliable transportation
- Auto insurance (state minimums)
- A smartphone for documenting visits in the field
Nice to have
- Bilingual Spanish/English (high patient demand in Southern NM)
- BLS/CPR certification, or willing to get it
- Dual Texas and New Mexico licensure
- Home health experience. New grads are welcome, with mentorship.
- OASIS documentation experience
