What You'll Do
As a Transitions Hospice Chaplain, you'll be the spiritual anchor of the interdisciplinary care team — connecting patients and families to the support they need, in the way that resonates most with them. Your responsibilities include:
- Assessing the spiritual needs of patients, families, and caregivers with sensitivity and timeliness
- Providing non-denominational spiritual care directly, or coordinating with preferred spiritual advisors and clergy
- Developing, documenting, and continuously refining individualized spiritual care plans with the interdisciplinary team
- Educating and consulting with care team members on spiritual and emotional support best practices
- Building relationships with local faith communities to expand the network of spiritual support available to patients
- Conducting funeral, memorial, and bereavement services as requested — including annual services for Transitions families
- Participating in on-call duties on evenings and weekends as needed
- Maintaining accurate records in compliance with local, state, and federal requirements
- Showing up as a positive, motivating presence for colleagues and the broader care team

Requirements
What You Bring
- Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree), or degree in counseling, or equivalent experience.
- Comfort and experience providing non-denominational care across diverse faith backgrounds
- A calm, grounded presence in emotionally intense situations
- Strong interpersonal skills and the ability to collaborate within a clinical interdisciplinary team
- Valid driver's license and willingness to travel locally for patient visits
Salary : $26 - $29
