DESCRIPTIONTransitions Healthcare is seeking to hire a qualified, experienced and compassionate Wound Care Nurse Practitioner to join our dedicated team. Transitions Healthcare is a Florida-based mobile medical provider dedicated to supporting patients wherever they call home including in their home, skilled nursing, rehabilitation, and long-term care facilities. As a Nurse Practitioner, you will play a crucial role in providing a variety of patient visits, ranging from Transitional Care Visits to Wound Care Visits while assisting in limiting rehospitalizations and providing comprehensive wound care services.Responsibilities:Conduct in-home and community-based patient visits, providing comprehensive healthcare servicesDeliver advanced wound care including but not limited to dressing changes, debridement and skin graft applicationsDevelop and implement customized treatment plans for patientsDocument visits on electronic medical recordDeliver exceptional patient care, ensuring the best possible serviceRequirements:Active Florida Nurse Practitioner licenseMinimum 1 year experience as a ARNPMinimum 1 year experience in wound careWound care certification preferredAbility to travel to multiple locations throughout Volusia County, FLAbility to work independently and collaborate with all members of the health care teamCompetent with technology and electronic medical recordsBenefits: Malpractice insurance providedAnnual CEUs providedNo weekends, nights, call or holidaysFull time and Independent contractor opportunities available at other locations throughout Florida
