Transitions Healthcare is seeking to hire a qualified, experienced and compassionate Wound Care Nurse Practitioner to join our dedicated team. Transitions Healthcare is a Florida-based mobile medical provider dedicated to supporting patients wherever they call home including in their home, skilled nursing, rehabitilation, and long-term care facilities. As a Nurse Practitioner, you will play a crucial role in providing a variety of patient visits, ranging from Tansitional Care Visits to Wound Care Visits while assisting in limiting rehospitalizations and providing comprehensive wound care services.
Responsibilities:
- Conduct in-home and community based patient visits, providing comprehensive healthcare services
- Deliver advanced wound care including but not limited to dressing changes, debridements and skin graft applications
- Develop and implement customized treamtment plans for patients
- Document visits on electronic medical record
- Deliver exceptional patient care, ensuring the best possible service
Requirements:
- Active Florida Nurse Practitioner license
- Minimum 1 year experience as a ARNP
- Minimum 1 year experience in wound care
- Wound care certification preferred
- Ability to travel to multiple locations throughout Volusia County, FL
- Ability to work independently and collaborate with all memebers of the health care team
- Competent with technology and electronic medical records
