Transitions Healthcare North Huntingdon is hiring a Nurse Assessment Coordinator! This position is open to both RNs and LPNs.

As our Nurse Assessment Coordinator, you’ll work closely with our MDS Coordinator to plan and carry out MDS completion, following facility procedures along with state and federal regulations. Accurate assessments are what make sure every resident gets the right care plan and the support they need, so your attention to detail will have a real impact on their day-to-day lives.

What You’ll Do

• Gather the data needed to complete the MDS
• Help identify significant changes in residents’ conditions
• Monitor resident care plans for accuracy and completeness
• Attend the facility’s Medicare meeting
• Help educate nursing staff and the interdisciplinary team on the MDS process
• Enter MDS information into the system
• Review messages from our contracted pharmacy about medications that need prior authorization

What We’re Looking For

• Current Pennsylvania LPN or RN license (required)
• CPR/AED certification (required)
• At least 1 year of nursing experience (required)
• Proficient in Microsoft Office Suite and Outlook
