What are the responsibilities and job description for the Case Manager position at Transitions Counseling and Development Center?
Overview
We are seeking a dedicated and Reliable Case Manager to join our team and oversee a variety of administrative tasks for our organization under a LPC . This includes but not limited to conduct intakes and complete documentation for Outpatient Treatment request for treatment authorizations of clients to obtain and maintain services, scheduling, quality assurance, participates in community outreach programs and resource activities.
Responsibilities
- Coordinate patient intake processes and schedule appointments while ensuring accurate medical records and documentation
- Assist with Providing administrative support at he front desk, including Multi-line phone handling and client service
- Manage discharge planning processes, including arranging follow-up services and community resources.
- Utilize Electronic Health Record (EHR) systems and Electronic Medical Record (EMR) systems to document client interactions
- Perform utilization review and management to ensure appropriate resource use and compliance with LDH regulations such as HIPAA.
- Collaborate with clinical staff to ensure smooth outpatient workflows and quality assurance for documentation.
- Support intake procedures by gathering patient information efficiently and empathetically to initiate appropriate care pathways. Requirements

Pay: $15.00 - $18.00 per hour
