The Case Manager of Day Habilitation completes MassHealth Day Habilitation regulatory paperwork and ensures that all regulatory paperwork is completed, consumer satisfaction is high, and that team meetings, Day Hab Service Plan’s, and Interdisciplinary Team meetings are held on a regular basis according to regulatory guidelines. Duties include:
Assessment, Planning, and Development
- Ensure that staff is familiar with Transitions Centers mission and vision and follow them accordingly.
- Ensures collaborating relationships with all involved parties, including funding sources, state agencies, families, other providers, staff and members.
Reporting and Documentation
- Ensure that all required documentation is completed in conjunction with all MassHealth Regulations.
- Reports to the Director of Transitions Centers on a monthly basis, the status of Day Habilitation regulatory paperwork.
- Ensure that all Member files are updated and completed per MassHealth regulations.
Administrative Duties
- Attends all referral meetings, ensuring that all paperwork required and submitted and then brings packet and presents at Interdisciplinary Team meeting.
- Attends all referral interviews submitting final candidates to the Manager of Day Habilitation.
- Coordinates ancillary program service needs (i.e. transportation, employment, individual support services).
- Coordinate acceptance and discharge of members.
- Leads Interdisciplinary Team meetings, creating all relevant paperwork
- Complete all member annual Day Hab Service Plan reports with assistance from the Manager of Day Habilitation.
- Oversee Individual Service Support Plan for members receiving 1:1 support and ensure completion of assessments, goals, semi and annual reports according to MassHealth timelines/regulations.
- Direct Member Care
- Fill in for direct care staff when the ratio is not met/as needed.
40 hours per week. Monday-Friday 8:00am-4:00pm
