Transitions Buprenorphine Clinic is looking for a full-time Substance Abuse Navigator to work in our Sacramento clinic.
Substance use navigator (SUN) helps patients access evidence-based treatment for substance use disorders with a focus on low-threshold access to MAT (medication addiction treatment) and harm-reduction. SUNs' duties are to screen for candidates upon intake and conduct initial brief assessments and serve as a resource coach for patients and patient advocates during their treatment. They also assist with access to other services such as financial counseling. primary care, and social services.
Responsibilities:
- Establish a positive relationship with past and new patients within the clinic.
- Promote communication between patients and the clinic via telephone, texting, patient portal, and/or email to remind patients of appointments.
- Help patients navigate any obstacles to follow-up treatment, and provide encouragement.
- Use motivational interviewing techniques to communicate with patients in a respectful culturally appropriate, non-judgmental manner.
- Maintain up-to-date information about the effects of various substances, withdrawal symptoms, and treatment options in order to effectively educate and counsel patients.
- Promet HRS based on patients' goals, preferences, and life circumstances.
- Assist with navigating barriers to patients obtaining buprenorphine prescription from pharmacy.
- Develop connections with a comprehensive array of community service providers to address the needs of people with SUD or co-occurring mental health disorders.
- Provide patients referrals to other services such as shelter, primary care, mental health services, social services, and residential treatment facilities.
Qualifications:
- Ability to collaborate effectively with mental health, alcohol, drug, and other human service providers.
- Must be able to demonstrate the capacity to form a therapeutic alliance with co-occurring disorder patients.
- Flexibility and creativity in application of treatment and recovery approaches.
- Interest/proficieny in working with individuals recently released from incarceration, homeless individuals, and other marginalized populations.
- Nonjudgemental, energetic, positive approach to assisting patients with Substance Use Disorder.
Job Type: Full-time
Pay: $25.00 - $32.00 per hour
Expected hours: 40 per week
