Transitions At Home, is currently seeking to hire a Full-Time, experienced and compassionate RN Clinical Manager to join our Home Health team! This position provides clinical leadership and support to our nursing and therapy staff while ensuring quality patient care and compliance with all applicable standards. What does a Transitions at Home RN Clinical Manager do?
Transitions At Home RN Clinical Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction of the Administrator.

Hours: Full-Time
- 40 hours per week 8:00 am - 4:30pm
- Monday through Friday
- Limited on-call rotation

Qualifications / Job Requirements:
- Active Wisconsin Registered Nurse License
- Three to five (3-5) years’ nursing experience, 1 year in Home Health (preferred)
- Ability to supervise, guide and develop nursing skills and performances
- Participates in the development of standards which ensure safe and therapeutically effective service to patient/family's.
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
