What are the responsibilities and job description for the Intake & Care Coordination Specialist position at Transitional Services Inc?
The Intake & Care Coordination Specialist is responsible for managing all referrals to the Permanent Supportive Housing Project and TAY Program including vacancies, waiting list, and interviewing applicants for determination of eligibility for the program. The Intake & Care Coordination Specialist is responsible for managing all aspects of the referral and housing databases, generating reports for funders, and gathering and disseminating housing resource information to stakeholders. The Intake & Care Coordination Specialist is responsible for coordinating care with other service providers and providing clinical support and training to staff. In conjunction with other team members, the Intake & Care Coordination Specialist assists in ensuring that daily operations within the programs are consistent with TSI's mission statement and guiding principles as well as the agency's Psychiatric Rehabilitation Approach Mission statement and values.
Schedule is Monday through Friday daylight
$1,000 sign-on bonus
- Master's degree in social work, psychology, or a health-related field
- One year clinical experience in MH or SUD
- Fluency in use of databases, spreadsheets and web-based applications
- Minimum tow years experience in mental health
- Licensed Professional Counselor or Licensed Clinical Social Worker a plus
- Valid PA driver's license
