Responsibilities:

Responsibilities:
- Facilitates rehabilitation of patients with emotional, physical, or mental disabilities.
- Implements speech therapy care plans.
- Coordinates care with entire team.
- Educates patients and family members.
- Prepares timely, complete, and accurate clinical notes.
- Communicates effectively with patients, caregivers, healthcare providers, office support and other care team members.
- Speech language pathologist in the State of Texas.
- Home health experience highly preferred.
- Bilingual (Fluent in Spanish) encouraged to apply.
- Experience with Home Care Home Base EMR (preferred but not required).
- Knowledge of home health regulations and general healthcare compliance.
- Ability to handle physical workload.
- Good driving record.
- Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
