Responsibilities:
- Assist in the facilitation of rehabilitation of patients with emotional, physical or mental disabilities.
- Assist in the implementation of physical therapy care plans.
- Assist with the coordination of care with the home healthcare team.
- Educate patients and family members about health conditions.
- Prepare timely, complete, and accurate clinical notes.
- Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
- Assistant Physical Therapist in the state of Texas.
- Home health experience highly preferred.
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR. (preferred but not required)
- Able to handle physical workload.
- Good driving record.
- Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
