- Create, maintain, and update the clinicians' schedules according to authorizations and patient needs.
- Maintain clinicians' availability list.
- Serve as the point of contact for all interactions with clinical team members and administrative associates in relation to scheduling
- Maintain a tracking system to ensure that all visit frequencies are input for proper scheduling
- Work with DON to ensure that all clinicians meet productivity.
- Assists with changes in staff schedules and reassigns patients appropriately.
- Maintaining positive working relationships with internal and external customers, while exhibiting the core values of Transition Home Healthcare.

- 3 years' experience with healthcare scheduling (Home Health)
- Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- Minimum 3 years of hands-on scheduling experience within HomeCare HomeBase (HCHB) required
- Strong understanding of Medicare home health visit frequency management, episode timing, and scheduling compliance requirements
- Experience coordinating both employed and contract clinical staff across multiple disciplines including SN, PT, OT, SLP, HHA, and MSW
- Experience managing scheduling operations in a high-volume Medicare-certified home health agency strongly preferred
- Ability to interpret physician orders, visit frequencies, care plans, and authorization limitations within scheduling workflows
- Experience utilizing scheduling dashboards, workflow queues, and operational reporting within HCHB
- Having a medical license and background (LVN or RN) would get a huge plus!
