Job Description Summary

The Ambulatory Primary Care Nurse position is a clinical role in the Primary Care department and reports to the Nurse Manager. This position is salaried, union, full-time, and on-site.

The Ambulatory RN is responsible for providing exceptional adult primary care, pediatric primary care, and mental health services to gender diverse patients.

Objectives & Responsibilities

Clinical Duties (90%)

  • Ensure safe, high quality care for patients and their families
  • Holistically assess the physical, psychological, social, and cultural dimensions of patients according to professional practice standards and institutional policy/procedure
  • Provide clinical care within RN scope of practice and in accordance with provider orders including but not limited to: patient education, vaccine and medication administration, injection training, wound care, suture and staple removal, ear irrigation, sterile procedure set up, splinting/bracing, and collection of lab specimens.
  • Employ excellent clinical reasoning for phone triage.
  • Develop culturally competent plans of care that identify patient problems, expected outcomes, and address preventative measures
  • Develop and implement patient/caregiver teaching plan based on identified health education needs
  • Maintain detailed, timely records in the electronic health record and medication documentation logs.
  • Provide electronic medical record inbox support to providers including patient triage, referrals communication, education, prescription management, utilization of routine nursing treatment protocols.
  • Organize, prioritize, and coordinate nursing care activities considering the needs of patients and staff
  • Implement patient population specific interventions and measure outcomes
  • Demonstrate leadership in the delivery of patient care, proactively identifying and seeking solutions to issues that arise.
  • Foster and encourage clinical team cohesion.
  • Work to create an affirming and inclusive environment for all patients and staff.
  • Assist in coordinating clinical response to emergencies.
  • Other duties as assigned

Administrative (5%)

  • Collaborate with administrators and clinical staff to plan, implement, and evaluate pertinent changes in practice and health care delivery
  • Develop and implement nursing work flows and policies in collaboration with the leadership team, and in accordance with state and federal laws/policies.
  • Participate in ongoing quality assurance programs
  • Attend staff meetings
  • Other duties as assigned

Educational (5%)

  • Participate in the development of formal and informal educational programs for the staff
  • Assume responsibility for own professional development and contribute to the professional development/training of others
  • Maintain current licensure and certifications required for employment
  • Support research efforts to advance knowledge and to promote research-based practice

Qualifications & Experience

  • Graduate of an accredited School of Nursing within the United States
  • Registered Nurse currently licensed in the State of Massachusetts
  • Basic Life Support Certification required
  • Three years experience as an RN preferred
  • Experience in primary care preferred
  • Experience in triage, preferably for adult/pediatrics in an ambulatory setting preferred
  • Past experience working with transgender/gender diverse community is a strong asset

The employee frequently is required to use hands or fingers, handle or feel objects, tools, or controls. The employee is occasionally required to stand; walk; sit; and reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus. The noise level in the work environment is usually low to moderate. There are no adverse environmental conditions expected.

Pay: $88,000.00 - $107,160.00 per year
