Outpatient Counselor LADC or ADC-T-Hybrid Position
Transformation House - Recovery Hub
Are you passionate about helping others and making a lasting impact? Recovery Hub, part of Transformation House is seeking a dedicated Licensed Alcohol and Drug Counselor (LADC) or Alcohol and Drug Counselor – Temporary (ADC-T) to join our supportive, multidisciplinary team in our outpatient program for a hybrid position, doing both telehealth and in-person groups.
Responsibilities:
- Conduct comprehensive substance use assessments
- Provide case management and individual sessions
- Facilitate therapeutic group sessions
- Perform the 12 core counseling functions
- Utilize the Minnesota Matrix Six Dimension model
- Maintain accurate, timely documentation
- Collaborate in weekly multidisciplinary team meetings
Qualifications:
- Current MN LADC license or ADC-T temporary permit
- Ability to pass a DHS background study
- Experience with EMR (Procentive preferred)
- Basic computer skills
- 12 hours of co-occurring training within 6 months of hire (8 hours per year ongoing)

Pay: $65,000.00 - $75,000.00 per year
