Job Summary:
The Hospice Chaplain provides spiritual, emotional, and bereavement support to patients, families, and caregivers in accordance with the hospice philosophy of care. The chaplain serves as a core member of the Interdisciplinary Group (IDG), ensuring that spiritual and cultural needs are assessed and addressed as part of the overall plan of care. This position also collaborates with community clergy and provides education and support for staff and volunteers regarding spiritual care.
Essential Duties & Responsibilities Patient & Family Support
- Conduct initial and ongoing spiritual assessments of patients and families.
- Develop individualized spiritual care plans in collaboration with the IDG.
- Provide pastoral counseling, prayer, sacraments, rites, or rituals as requested.
- Offer end-of-life support, crisis intervention, and grief counseling.
- Respect and support diverse spiritual, cultural, and religious traditions.
Collaboration & Interdisciplinary Team
- Participate in IDG meetings and contribute to the holistic plan of care.
- Coordinate with social workers, bereavement coordinators, and volunteers for comprehensive family support.
- Maintain relationships with community clergy to support patient/family preferences.
Documentation & Compliance
- Document visits, interventions, and care plans accurately and timely in the EMR.
- Ensure compliance with CMS Conditions of Participation, Georgia Department of Community Health standards, HIPAA, and agency policies.
- Track patient and family outcomes related to spiritual care.
Education & Support
- Provide grief and spiritual education for hospice staff, volunteers, and community groups.
- Facilitate or participate in memorial services, remembrance events, and community outreach.
Qualifications
- Education:
- Master of Divinity (M.Div.) or equivalent degree from an accredited theological seminary preferred.
- Clinical Pastoral Education (CPE) units strongly preferred.
- Credentials:
- Endorsement/ordination from a recognized faith group required.
- Board Certification through APC, NACC, NAJC, or CPSP preferred (not always required).
- Experience:
- At least 1–2 years of pastoral/spiritual care experience required.
- Hospice, palliative care, or healthcare chaplaincy experience preferred.
- Skills:
- Strong counseling, listening, and crisis intervention skills.
- Ability to work with diverse populations with respect for all spiritual and cultural traditions.
- Ability to travel to patient homes, nursing facilities, and hospitals as needed.
- Proficiency in Microsoft Office and hospice EMR systems.
Work Conditions
- Schedule: Full-time or PRN; includes on-call rotation, evenings, weekends, and holidays as needed.
- Setting: Patient homes, skilled nursing facilities, hospitals, and office environments.
- Physical Requirements: Ability to sit, stand, and travel frequently; occasional lifting up to 25 lbs.
