Position Title

Direct Support Professional (DSP)

Position Summary

The Direct Support Professional (DSP) provides high-quality, person-centered care and supervision to residents of Tranquil Pathway Homes. The DSP supports residents in activities of daily living, health and wellness needs, and community engagement while promoting independence, dignity, choice, and safety. This role requires collaboration with team members, healthcare providers, and families to ensure compliance with Wisconsin Adult Family Home regulations and to maintain a safe, supportive, and respectful living environment.

Essential Duties and ResponsibilitiesGeneral Responsibilities

  • Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene.
  • Administer medications and treatments in accordance with training, physician orders, and individualized service plans.
  • Prepare and serve meals in compliance with dietary requirements and nutritional guidelines.
  • Monitor, observe, and accurately document resident health status, behaviors, and care activities.
  • Assist with mobility, transfers, and proper positioning using safe body mechanics.
  • Maintain a clean, safe, and organized home environment, including laundry, light housekeeping, and infection-control practices.
  • Support resident participation in social, recreational, and community-based activities.
  • Communicate effectively with families, case managers, healthcare providers, and interdisciplinary team members regarding resident care.
  • Follow all Adult Family Home policies, procedures, and Wisconsin DHS regulations, including mandatory reporting requirements.
  • Participate in required training, continuing education, and staff meetings.
  • Maintain confidentiality of resident and organizational information in compliance with HIPAA.

Shift-Specific ResponsibilitiesDay / AM Shift

  • Assist residents with morning routines, personal care, and dressing.
  • Administer morning and mid-day medications as scheduled.
  • Prepare and serve breakfast and lunch.
  • Coordinate and facilitate daily recreational, therapeutic, and community activities.
  • Complete Compass IL documentation and all required care records.
  • Schedule and coordinate resident appointments and outings.
  • Communicate resident updates with families, case managers, and interdisciplinary teams.
  • Complete grocery shopping, household supply procurement, and meal planning.
  • Ensure documentation is current, accurate, and compliant with agency standards.

Evening / PM Shift

  • Prepare and serve dinner.
  • Assist residents with showers, evening personal care, and bedtime routines.
  • Engage residents in evening leisure and recreational activities.
  • Administer evening medications as required.
  • Perform household cleaning and organization, including garbage removal.
  • Complete evening documentation and Compass IL logs.
  • Ensure residents are safely in bed and the home is secured for the night.

Nocturnal / NOC Shift

  • Provide overnight supervision and monitoring to ensure resident safety.
  • Assist with nighttime personal care, toileting, or medications as needed.
  • Perform meal preparation or setup for the following day when required.
  • Prepare and organize the home for morning routines.
  • Complete overnight documentation, including Compass IL and shift logs.
  • Respond promptly and appropriately to resident needs, alarms, or emergencies while maintaining a calm and supportive environment.

Qualifications

  • High school diploma or equivalent required.
  • CNA certification or equivalent caregiving training preferred but not required.
  • Previous experience in caregiving, healthcare, or residential support settings preferred.
  • Current CPR and First Aid certification, or willingness to obtain upon hire.
  • Ability to safely lift, transfer, and assist residents.
  • Strong communication, interpersonal, and problem-solving skills.
  • Knowledge of infection control, safety procedures, and medication administration principles.
  • Successful completion of background checks, TB testing, and all DHS-required pre-employment screenings.

  • Flexibility to work varying shifts, weekends, and holidays.

Physical Requirements

  • Ability to lift, carry, and assist residents weighing up to 50 pounds.
  • Ability to stand, walk, bend, and perform physical tasks for extended periods.
  • Ability to complete household tasks such as cleaning, laundry, and meal preparation.
  • Ability to respond quickly and effectively to emergencies and resident needs.

Work Environment

  • Residential Adult Family Home setting.
  • Regular exposure to household cleaning products and personal care tasks.
  • Frequent close interaction with residents, families, and healthcare professionals.

Compliance and Safety Requirements

  • Adhere to all Wisconsin DHS and Adult Family Home regulations and policies.
  • Immediately report suspected abuse, neglect, or safety concerns in accordance with mandatory reporting laws.
  • Maintain strict confidentiality of resident and organizational information in compliance with HIPAA.
  • Participate in all mandatory training and continuing education program

Compensation and Wage

  • Hourly wage range: $16.50 per hour, based on experience, training, certifications, and shift assignment.
  • Shift differentials may apply for evening, overnight (NOC), weekend, and holiday shifts.
  • Opportunities for wage increases based on performance, tenure, and additional responsibilities.

Pay: From $16.50 per hour
