Job Title: Social Worker MSW / LSW - Home Health and Hospice
Job Location: Our Social Workers see patients in the most ideal setting - the patient's home!
Our office is located in Dunmore, PA 
Shift(s): Full-time - Office hours Mon - Fri 8:30 - 5:00.

Duties and Responsibilities include, but not limited to the following:

• Assess patient care and plans with patient, family, and physician.

• Assist the patient with ADL and facilitate their efforts towards self-sufficiency and independence.

• Assess home environment regarding self-care, coping, and living arrangements.

• Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (social services, area on aging, meals on wheels etc.) to carry out treatment plan objectives.

Requirements:

• Master's Degree in Social Work
• Current Pennsylvania LSW / LCSW preferred

• Must have a current driver's license

• Ability to travel to case assignments as assigned

• Ability to work independently with minimal supervision and manage time effectively

• Strong interpersonal skills
