Your Role as an RN Case Manager:
- Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
- Provide hands-on care, administer medications, and evaluate patient progress.
- Maintain and submit timely documentation, including OASIS assessments.
- Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
- Educate patients and families on treatment plans and self-care techniques.
- Inform patients of additional services and resources as needed.
What You Need to Succeed:
- Active RN License in Pennsylvania
- BLS Certification
- Valid Driver’s License & Reliable Transportation
- Strong Assessment & Documentation Skills
- Ability to Work Independently & Manage Time Effectively
- Excellent Communication & Interpersonal Skills
- Acute Care and/or Home Health Experience (Preferred)
