Duties and Responsibilities include, but not limited to the following:
- In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
- Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
- Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
- Participate in quality improvement activities as assigned
- Timely and accurately document (EMR) all patient assessments, interactions, etc
- Must possess a current, valid Pennsylvania PTA license and have current CPR certification
- Valid driver's license and ability to travel to case assignments
- Must be comfortable working in home setting
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal and communication skills
