Job Title: Licensed Practical Nurse
Service Location: Forest City, Pa; Carbondale, Pa; Peckville, Pa; Lake Ariel, Pa; Throop, Pa
Shift: Weekday and weekend shifts, full-time and part-time schedules available!
Rate: $28-$32 per hour
Immediate Need:
Forest City, Pa: Monday through Friday 6am to 8pm and Weekends 8am to 4pm

Responsibilities:
- One to One patient care in home/school environment
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members
