Job Title: LPN/RN
Job Location: Dalton PA; Scott Township, PA; Factoryville, PA;
Immediate Need:
Dalton, Pa - 7 days a week, days and evenings
Scott Township, PA - Monday through Friday 7:15am to 4:45pm
Factoryville, PA- 12 hour skilled nursing night shifts for 9pm to 9am
- 16 hour skilled nursing 3pm to 11pm and 11pm to 7am
Rates: $28.00-$32.00
Job responsibilities include, but not limited to the following:
- One to One patient care in home
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members
