Tradewater Health & Rehabilitation is a small, familyowned nursing home committed to meaningful change, higher quality standards, and a renewed focus on honoring the fact that our building is the home of our residents. We are seeking an experienced MDS Coordinator who is a licensed nurse (RN or LPN) and is passionate about accuracy, accountability, and residentcentered care.
This role is essential to ensuring that every resident receives the quality of care they deserve — and that our documentation reflects the respect we have for their home.
Key Responsibilities
- Lead the MDS/RAI process with accuracy, timeliness, and regulatory compliance
- Complete assessments that truly reflect each resident’s needs, strengths, and goals
- Collaborate with the interdisciplinary team to develop care plans that honor residents’ preferences and dignity
- Support quality improvement efforts and help drive positive change throughout the facility
- Monitor quality measures and ensure documentation supports highquality outcomes
- Participate in care plan meetings and communicate clearly with staff and leadership
- Promote a culture where documentation, accountability, and resident respect are nonnegotiable
Qualifications
- Active Kentucky RN or LPN license (required)
- Previous MDS/RAI experience (required)
- Strong understanding of PDPM, care planning, and regulatory requirements
- Commitment to quality, accuracy, and continuous improvement
- Excellent communication and teamwork skills
- Passion for supporting a residentfirst culture in a homelike environment
