Targeted Case Manager

Position Summary
We are seeking a compassionate and detail-oriented Case Manager to join our team. This role focuses on coordinating patient care, supporting treatment plans, and improving health outcomes within a fast-paced healthcare environment.
Key Responsibilities
- Assess patient needs and develop individualized care plans.
- Coordinate with providers across departments, including ICU and PICU.
- Maintain accurate documentation and medical records.
- Provide education and resources to patients and families.
- Support smooth transitions of care and discharge planning.
- Monitor patient progress and adjust care plans as needed.
- Advocate for patients to ensure timely access to services.
Qualifications & Skills
- Strong knowledge of patient services and case management processes.
- Familiarity with pediatrics, trauma care, or acute care settings preferred.
- Proficiency in medical terminology and documentation.
- Excellent communication and teamwork skills.
