Job Title: Intake Coordinator

Position Summary
The Intake Coordinator supports the admissions and assessment process at TPC Wellness by assisting with client intakes, gathering required documentation, completing screening tools within scope of certification, and coordinating information between clients and the clinical team. This position works under the supervision of licensed clinical staff and plays a key role in ensuring accurate, timely, and compliant intake processes. The ideal candidate demonstrates professionalism, organization, strong communication skills, and a commitment to client-centered care.
Essential Duties and Responsibilities
- Report directly to Operations Director
- Assists Community Outreach Director with scheduling transport for TPC individuals served.
- Assist clients with the intake and admission process in a respectful and supportive manner
- Review, explain, and assist with completion of informed consent documentation under supervision
- Obtain and process releases of information in compliance with HIPAA and 42 CFR Part 2
- Collect intake information and complete screening tools within scope of CDCA certification
- Assist licensed staff with gathering information for bio-psycho-social assessments
- Demonstrate basic knowledge of Substance Use Disorder (SUD) symptoms and treatment services
- Document client information accurately and timely in the electronic health record
- Observe and report client behaviors, mood, and engagement to licensed clinical staff
- Assist with risk screening and immediately report safety concerns to supervising clinicians
- Support the development and documentation of safety plans as directed by licensed staff
- Communicate intake and client-related information to appropriate team members
- Participate in treatment team meetings as requested
- Coordinate and communicate with other departments to support continuity of care
- Assist with insurance-related intake information as directed
- Communicate with probation officers or referral sources under supervision when applicable
- Attend and complete all required trainings, certifications, and continuing education
- Maintain organized intake documentation to support audits and regulatory compliance
- Assist other departments as needed to support program operations
- Perform all other duties as assigned by TPC management
Minimum Qualifications
- Active Ohio Chemical Dependency Counselor Assistant (CDCA) certification
- Must maintain current certification and comply with all credentialing requirements
- Valid drivers license with acceptable driving record
Education Requirements
- High school diploma or GED required
- Associate degree in Social Work, Health Services, or related field preferred
- Relevant behavioral health experience may be considered in lieu of formal education
