Role Description
The Registered Nurse Case Manager will provide comprehensive nursing care to hospice patients in Medina. This on-site, full-time role involves conducting patient assessments, developing plans of care, coordinating services among the care team, and providing education to patients and families. The nurse will monitor patient progress, ensure quality and compliance with hospice regulations, and offer compassionate emotional support to patients and their loved ones.
Qualifications
- Licensed Registered Nurse in good standing with the state board of nursing
- Ability to perform patient assessments, develop care plans, and provide direct patient care
- Experience in hospice or palliative care is highly preferred
- Strong communication and interpersonal skills to educate and support patients and families
- Organizational skills to coordinate interdisciplinary care and maintain accurate documentation
- Knowledge of hospice regulations, end-of-life care, and symptom management
- Current CPR certification
- Ability to work collaboratively in a team-oriented environment
- Reliable transportation and willingness to travel locally for patient visits
