General Statement of Duties
Leads marketing efforts and supports census growth while assisting with the administration of the admissions process for a skilled nursing facility providing short term rehabilitation and long-term care services. Reports to the Director of Admissions.
Qualifications
Education and Experience:
Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or a related field from an accredited college or university, plus three (3) years of admissions, marketing, or referral development experience in a health care setting.
Qualifications:
Demonstrated sales and marketing ability within a healthcare environment.
Working knowledge of skilled nursing services, rehabilitation therapy, and long-term care operations.
Demonstrated understanding of common medical conditions treated in skilled nursing facilities.
Demonstrated ability to build and maintain professional relationships with referral sources.
Demonstrated knowledge of Medicare, Medicaid, and managed care coverage requirements.
Demonstrated knowledge of regulatory requirements governing long-term care requirements.
Proven ability to organize, schedule and prioritize work.
Proven ability to work independently and exercise sound judgment.
Demonstrated ability to deal tactfully and courteously with residents, families, and healthcare professionals.
Proven ability in the use of a personal computer, Microsoft Office applications, and the ability to learn facility databases used to access patient and hospital records.

The examination to determine a candidate's eligibility for the position will be a Training and Experience Assessment. Duties and Responsibilities:
Builds and develops professional relationships within the Southern Fairfield County healthcare community to generate qualified referrals for short-term rehabilitation and long-term care services.
Maintains a structured territory outreach plan and conducts regular in-person visits to hospitals and referral sources.
Represents the facility at professional meetings, healthcare events, and community outreach activities to strengthen referral partnerships and promote events.
Monitors referral activity, tracks referral trends, and conversion rates, and identifies opportunities to increase admissions.
Supports initiatives to increase and maintain facility census.
Coordinates with medical, nursing, rehabilitation, and finance staff to ensure smooth transition for patient/resident admission.
Meets with prospective residents and families, conducts facility tours and explains available care options and services.
Responds promptly to referral inquiries and facilitates timely clinical review and admission decisions.
Reviews hospital discharge summaries, medication lists, therapy notes, and other clinical documentation to assess appropriateness for admission.
Coordinates the admission process to ensure the required documentation, clinical evaluations, and financial information are completed prior to admission.
Coordinates with medical, nursing and accounting staff to ensure smooth transition for patient/resident admission.
Follows up with residents and families after admission and upon discharge to support continuity of care and satisfaction, where appropriate.
Communicates with the hospital when residents are transferred for acute care and evaluates potential readmissions upon return.
Collaborates with the Director of Admissions and admissions staff regarding medical and financial considerations that impact upon admission decisions.
Conducts insurance verification including pre-certification and pre-authorization with payment sources.
Maintains current knowledge of Medicare, Medicaid, and managed care requirements.
Ensures all patient and resident information is handled in accordance with all HIPAA Privacy Regulations.
Prepares marketing activity and census reports.
Monitors inventories of supplies and marketing materials to ensure availability for outreach activities.
Performs related work assigned.
Supports Town policies and philosophies.
