Key Responsibilities
- Deliver individualized treatment and wound care in alignment with physician orders and established care plans.
- Assess, monitor, and document residents’ skin integrity, collaborating with the nursing and interdisciplinary teams to ensure optimal healing outcomes.
- Partner with physicians and licensed nurses to develop and modify treatment regimens that address both the physical and psychosocial needs of residents.
- Educate residents, families, and staff on skin integrity, wound prevention, and care best practices.
- Maintain compliance with all applicable federal, state, and local regulations, as well as company policies and procedures.
- Support a positive, teamwork-driven environment that upholds Touchstone’s mission and values.
- Current and valid Texas RN or LVN license, or a Compact State RN/LVN/LPN license in good standing.
- Prior experience in skilled nursing, wound care, or long-term care preferred.
- Strong clinical judgment and attention to detail.
